-Long-tern use of metformin may be associated with biochemical vitamin B12 deficiency, and

periodic measurement of vitamine B12 levels should be considered in metformin-treated patients,

especially in those with anemia or peripheral neuopathy B

 

-In patients with long-atandind suboptimally controlled type 2 diabetes and established  atherosclerotic cardiovascular disease, empaglifllozin or liraglutide should be consided as they have been shown to reduce cardiovascular and all-cause mortality when added to standard care. Ongoind studies are investigating the cardiovascular benefits of other agents in those drug classes. B
